This is a preferred pricing plan and is not dental insurance. Your MyCare Dental Plan may not be combined with any other discounts or dental insurance. All fees subject to change. All MyCare members must live in same household. Annual membership fees are non-refundable and must be paid in full before enrollment begins. If payment is required for additional services, it is due in full at the time of service. The effective date of the plan is the date you enroll and is in effect for 12 consecutive months. Cleanings must be completed before the end of the coverage date. Membership is limited to Denzinger Family Dentistry. If you must be referred to a specialist, preferred pricing may not apply at that location.
